I. Adoptive parents must submit in person their child’s application to the Embassy of Viet Nam.
The following documents are required to submit:
(1) Application with photo (parents may sign on behalf of their child) (download form)
(2) 02 (two) colour photos (size 4 x 6 cm, taken within 1 year, without hat or sunglasses, white background, no covering ears).
(3) Current Vietnamese passport
(4) Original and a copy of Vietnamese Birth Registration
(5) Original and a copy of parents’ passports (page showing personal details).
(6) Original and a copy of Adoption Decision granted by Vietnamese local authority.
II. If you are unable to come in person or you are living in the Republic of Ireland, you may submit by post and all required documents must be notarised by Notary Public and then certified by UK Foreign and Commonwealth Office or Irish Department of Foreign Affairs and Trade.
The following documents are required for submission :
(1) Passport renewal application with photo (parents may sign on behalf of their child). (download form)
(2) 02 (two) colour photos (size 4 x 6 cm, taken within 1 year, without hat or sunglasses, white background, no covering ears).
(3) Current Vietnamese passport
(4) A certified copy of Vietnamese Birth Registration
(5) A certified copy of parents’ passports (page showing personal details)
(6) A certified copy of Adoption Decision granted by Vietnamese local authority
(7) 01 self-addressed envelop
Note
If you apply by post, you must pay postage fee for return. Each applicant’s required documents must be posted in separate envelop. Please do not pack either more than 1 applicant’s required documents or documents of different consular service in one envelop.
Your application cannot be processed if your required documents do not meet our requirements.
